Insurance Plans in which we participate:

If your insurance company is one of these, we will bill them directly for you;  if we are provided with a copy of your insurance card.

If we do not participate with your insurance plan, you will  work with  your insurance company to determine your reimbursement.  You will need to  submit your bill directly to them.
